HUSSAINI ASSOCIATION OF CALGARY Sunday School Program – Liability Waiver, Assumption of Risk, Indemnity, and Medical Release Form

1. Acknowledgment and Assumption of Risk
I, the undersigned parent/guardian, acknowledge and agree that my child’s participation in activities of Hussaini Calgary Sunday School, a program of the Hussaini Association of Calgary (HAC), involves inherent risks. These may include, but are not limited to: slips, falls, physical injury, illness, food-related issues, outdoor/environmental hazards, accidents during transportation to/from activities, or emotional/psychological stress associated with group learning.

I fully accept and assume all such risks, whether foreseeable or unforeseeable, known or unknown, that may arise from my child’s participation.

2. Waiver and Release of Liability
In consideration of my child being permitted to participate, I hereby voluntarily release, forever discharge, and agree not to sue:
• The Hussaini Association of Calgary (HAC),
• Hussaini Calgary Sunday School,
• Their officers, directors, teachers, employees, volunteers, contractors, representatives, and agents,

from any and all claims, liabilities, demands, actions, or causes of action, whether in law or equity, arising out of or relating to my child’s participation in any school activity.

This release applies to any claims for injury, illness, accident, property damage, emotional distress, or wrongful death, regardless of cause, except where directly caused by gross negligence or intentional misconduct.

3. Indemnification
I further agree to indemnify, defend, and hold harmless the Hussaini Association of Calgary, its affiliates, and its representatives from and against any claims, damages, liabilities, costs, or expenses (including legal fees) that may result from my child’s actions, negligence, or participation in school activities.

4. Medical Authorization
In the event of an accident, injury, or medical emergency involving my child, I authorize the Hussaini Calgary Sunday School staff or volunteers to:
• Administer first aid as necessary.
• Obtain emergency medical treatment for my child, including transport to a hospital or clinic.
• Contact me immediately using the information provided above.

I assume full financial responsibility for all medical expenses incurred on behalf of my child.

5. Governing Law and Severability
This agreement shall be governed by and construed under the laws of the Province of Alberta. If any provision of this agreement is found to be invalid or unenforceable, the remainder shall continue in full force and effect.
